Early findings include mottling of the RPE and blunted foveal reflex. While today it is used to treat autoimmune conditions, such as rheumatoid arthritis and lupus, it was originally used as an anti-malaria drug. The maximum dose of Plaquenil is 6.5 mg/kg of ideal body weight per day, regardless of whether a person is overweight, underweight, or normal weight. One kilogram kg equals 2.2 pounds. Visual Fields and Plaquenil Medical Billing and Coding. Our office has been including a second diagnosis for the disease related to Plaquenil use. We aren't seeing denials using that method but from a coding standpoint I don't know if that is a 'more correct' coding or if we are just billing different insurances and getting different responses regardless of our using a second dx code.
